The hospice medical director will provide consultation and direct symptom management as requested by the attending physician and when the attending cannot be reached. You may also choose to have the Hospice Medical Director serve as your attending physician.

Medical Director
Skilled Nurse (RN, LPN/LVN)
Your hospice nurse will provide you and your family with ongoing education throughout the progression of your terminal illness. The nurse will assist with providing symptom management, educating you and your caregiver, complete assessments, and work as a case manager to collaborate care with the interdisciplinary care team to update the plan of care as needed.
Hospice Aide
Your hospice aide will provide you assistance with activities of daily living including bathing, dressing, personal hygiene, eating, and toileting. The aide will assist your caregiver on transfers and will also provide non-pharmacological symptom management.
Medical Social Worker
Your social worker will provide you and your caregiver with the resources necessary to ensure that you receive the best care. The social worker will assess emotions related to the terminal illness, provide counseling services as needed, and provide you and your caregiver with financial and community resources as needed.
Chaplain
Your chaplain will provide you and your loved ones with any spiritual care that you might need along the way. The Chaplain will conduct religious services of prayer, worship, and rituals as you or your loved one’s request.
Volunteer
Your hospice volunteer will provide care and activities that support comfort management in the home setting. The volunteer can help by grocery shopping, preparing meals, transportation to appointments, and light housework.
Medical Supplies
Our agency will provide you with the supplies needed for your care. Supplies include but not limited to incontinence supplies, wound care supplies, oral care supplies, bathing supplies, hospital bed, bedside commode, walker, wheelchair, and any other supplies that apply to the care of your terminal illness.
Medications
Our agency will provide you with the medications necessary for pain and symptom management related to the terminal illness and other related conditions.
Medical Equipment
Our agency will provide you with durable medical equipment such as bed, oxygen, and assistive devices based upon your physical needs.
Bereavement Support
Our agency provides resources that help coping with grief before and after the loss of loved ones. This service provides up to 13 months of follow up services for hospice families.
